Instructions for downloading and completing the Repair Forms

Click on either of the download links (Wood or Iron Repair Forms).

Choose a location to save the file to on your PC.

The form is an executable form... just double-click it to run.

Enter the information on each club (you can type in the fields).

Choose "File Save As", giving it a name such as your name.
